Dr. Uday. Daphtary is a Family physician and certified by the state of Florida to evaluate patients with chronic conditions that would benefit from recommendation of medical marijuana . He delivers compassionate care to improve their quality of life through this alternative therapy.
Eligibility
- Patient must be a Florida resident with a valid Florida I.D or other proof of residency.
- Patients under 18 years of age must have a parent or guardian present during consultations and doctor’s appointments and must be approved for this treatment by two different physicians so as to get approved.
- Diagnosis of a medical condition approved by the state.
- Patient must have medical records of previously been treated for same condition prior to receiving a medical marijuana recommendation.
